Cotton Fantasy plays much like previous games in the series, with its systems being mostly carried over from Cotton Reboot!. In addition to the traditional side-scrolling stages the game also have behind-view on-rails levels similar to Panorama Cotton and Rainbow Cotton. The Jewel Fever mechanic from Reboot was reworked, with items now dropping from enemies whenever they are hit with magical spells.
